Go to our Contact Us Page and fill in the form. A team member will contact you to discuss your requirements.

Option Three: Email or Fax Maintenance Form

• Print the form out
• Fill in all the required details including the serial numbers of the equipment
• Fax the completed document to BodyTastic on (03) 9894 8766 or scan an image of the completed document and email it to service@bodytastic.com.au
